Cobb County School District website now has a special page for "School Health". It can be directly accessed at:

It includes the following information:

1. Forms for medication at school are under "My Student Needs Medicine at School."

2. Medical Forms for Asthma, Diabetes, Seizures, Allergies and Chronic Illness.

3. Parent Information Links to the CDC (Centers for Disease Control and Prevention), Children's Healthcare of Atlanta, Cobb County Health Department locations and Hospital/Homebound Services.
